Please note that AXA Equitable Life Insurance Company no longer markets health insurance coverage.  The Company believes each of its health insurance plans is a "grandfathered health plan" under the Patient Protection and Affordable Care Act (the 'Affordable Care Act").

Beginning September 23, 2012, the Affordable Care Act will require health insurers to provide a summary of certain health insurance plan's benefits and coverage in a document, following a prescribed template.  Please note that since this template is based on policy design and language used in policies issued today, it can differ significantly from the terminology used in this company's policies, all of which were issued over 20 years ago.  We recommend that you contact our Third Party Administrator at 1-800-782-9355 (STA-WELL) to answer any coverage and benefits questions that you may have, and/or to provide you with a copy of your policy.  You may view the uniform glossary at https://www.dol.gov/ebsa/pdf/SBCUniformGlossary.pdf or contact us to request a paper copy.
